Be it enacted by the Council as follows:
Section 1. Section 12-127 of the administrative code of the city of New York is amended by adding new subdivisions c and d, to read as follows:
c. Each agency shall keep records of workplace injuries or illnesses occasioned in the line of duty. Such records shall include, but not be limited to, the following data:
(1) the name of the person injured and/or filing a claim about such injury;
(2) such person's title;
(3) the date of such injury or illness;
(4) the location of such injury or illness;
(5) the name of the agency where such person worked and/or such injury occurred;
(6) the date such claim was filed, if any;
(7) the date such claim was established, if any;
(8) the length of time, if applicable, such person is out of work due to such injury or illness;
(9) the nature of such injury or illness; and
(10) a description of how such injury or illness occurred.
Each agency shall transmit such data to the law department. In the case of agencies whose employees are covered by workers' compensation or line of duty injury claims, such agencies shall also transmit such data to the office of management and budget.
d. The law department and the office of management and budget shall each compile and prepare a report from the data provided pursuant to subdivision c of this section. Such report shall also include, but not be limited to, (i) an analysis of each agency's workers' compensation expenses, including expenses related to wage replacement, medical costs, administrative costs and any penalties paid by such agency and (ii) an assessment of each agency's use of modified duty assignments and disability transfers. The reports shall provide year-to-year comparisons. Such report shall be transmitted to the department of records and information services pursuant to section 1133 of the charter. Such report shall also be transmitted each quarter to the mayor, the comptroller, the public advocate and each member of the council.
�2. This local law shall take effect immediately.
